Heritage Health fined for two resident neglect violations

Heritage Health of Peru has been fined $4,400 for a number of licensure violations involving two incidents of negligence that resulted in one resident’s death and another resident’s limb amputation, according to Illinois Department of Public Health documents.
On June 27, a certified nurse’s a­ide had pulled on a bed sheet causing an unidentified resident to roll off the bed and receive nasal fractures and bleeding within the brain. On July 4, the resident would ultimately die as a result of the injuries.
IDPH survey documents show the CNA did not immediately alert the nurse on staff of the fall, and instead, placed the resident back in bed and knowingly falsified reports on the fall until questioned about the severe bruising on the resident’s face.
The employee was immediately terminated, according to IDPH’s interview with administrator Caroline Daugherity.
“(The employee) should have told the nurse when (the resident) fell to the floor,” Daugherity stated in the IDPH report. “We did follow up and monitor this resident as the physician ordered. We involved the physician in all aspects of the resident’s care. We kept in constant contact with (the resident’s) family. I had to terminate (the employee). She lied to us about what happened and I could not trust her after that.”
n The IDPH report also indicated the facility did not follow its procedures on properly using full-body mechanical lifts after a CNA in October knowingly ignored policy and moved a resident improperly without a mechanical lift. The resident’s care plan stated the resident must be transferred using a mechanical lift and two personnel given the resident’s  osteoporosis and recent leg injury.
As a result of the improper move called a “pivot transfer,” the resident’s already broken leg sustained another fracture and the resident’s family was forced to expedite their decision to have the resident’s leg below the left knee amputated.
Heritage Health of Peru officials referred the NewsTribune to a corporate spokesperson who did not return phone calls by deadline this morning.

Very sorry to hear this. My Mother was a resident there for over 5 years and received nothing but the BEST care. I was constantly informed of any changes to her mood, condition, treatment, and daily care. Unfortunately, good news travels very slowly but news like this travels like wild fire. I have to think that these were very isolated instances. Please do not judge Heritage Peru by these two instances. They were beyond great with my Mother.
